Appointments
Our providers see patients by appointments which can be made by phone or in person. If you are a new patient, please fill out this New Patient Form in advance and bring it with you to your first appointment. This saves you time and allows you to have a longer, more in-depth conversation with the physicians when you come to our office. In addition, please bring a photo ID and your health insurance card(s) to your first visit, and any time that information is updated subsequently.
If you have previously had any studies performed outside of The Hand Center, P.C., such as X-Rays, MRI's, CT scans, EMGs, or ultrasounds, please obtain the images (CD preferable) and/or reports of these studies and hand carry them to our office for your first visit.
We understand your time is valuable and make every effort to see you on time. However sometimes emergencies occur which may delay your appointment.
As a courtesy, we respectfully request that you please refrain from wearing any cologne, perfume, scented lotions, or aftershave to your appointment. Your thoughtfulness and assistance is appreciated.
Please contact us to see if we are in network with your insurance plan. We gladly accept Workers Compensation and the following insurance plans:
Cancellations
If you need to cancel or reschedule your appointment, please notify us as soon as possible so we can accommodate other patients. We appreciate your consideration with this matter.
After Hours
If you have an urgent matter during after hours, the answering service will direct you to the appropriate number to call. Non-emergency matters should be addressed during regular office hours.
Referrals
If Dr. Lipton has recommended you receive additional studies (EMG/NCS, MRI, CT, Ultrasounds, etc.) our office will set up the referral for you. The providing facility will contact you to set up an appointment. Once you have completed the studies please call our office to set up a follow-up appointment with Dr. Lipton.
